Field	Description
Report Name	The name of the report.
Start Date	The start date from when the data must be considered for generating the report.
End Date	The end date until when the data must be considered for generating the report.

Old Tariff	The name of the old tariff.
New Tariff	The name of the new tariff.
Old Tariff Rev	The revenue amount earned from old tariff.
New Tariff Rev	The revenue amount earned from new tariff.
Revenue Change Reason	The reason for change in package/tariff revenue.
